New Client Information Terms for Flawless Footcare.
Thank you for choosing me as your foot healthcare provider – I truly appreciate your support and will do my absolute best to provide you with exceptional health care solutions for many years to come.
To ensure I provide you and my other valued clients with the best possible care it is important that you understand and agree to the terms and conditions as outlined below:
24 Hours' Notice of Cancellations: I am a busy and professional health provider – all clients MUST accept and agree to Flawless Footcare’s cancellation policy – as failure to keep a scheduled appointment not only disrupts your treatment progress but also takes the spot of another client who may have been able to book an appointment in your place.
If you are unable to keep a scheduled appointment, you MUST give 24 hours' notice – otherwise a cancellation fee of 50% of the cost of the appointment you scheduled will be charged – no exceptions will be made.
Pay at the Time of Consult – consultation fees for all clients must be paid at the time of consultation.

​I provide a mobile service, and therefore on occasion I may be delayed because of unforeseen circumstances. I always endeavor to keep to appointment times, however, please be aware that my arrival time is booked within a 30-minute window (15 minutes before or after appointment time). If I am delayed longer than this, I will phone you to make you aware.

If when attending a booked appointment within the 30 minute window as detailed above, and the client is not at the address ready to receive treatment, a full charge will still be made.

An additional charge will be made for any specialist applications, ointments, paddings or dressings: this will be discussed and agreed with patients in advance.

Full disclosure of your medical history is required to ensure I am delivering appropriate and safe care and treatments. Failure to do so may affect the outcome and could potentially have a detrimental impact on your health. On occasion I may recommend that I refer clients to other healthcare providers. This will only be done if full consent is obtained from the client.
Privacy Policy
For the purposes of correct and full diagnosis and treatment, Flawless Footcare may need to refer your case to your GP or other relevant specialist. This will always be done with your prior signed consent. Flawless Footcare will never share your information with a third party for marketing purposes or any reason unrelated to your specific relevant treatment without your clear consent.

Flawless Footcare retains basic personal information and all relevant treatment information on its patients for a minimum of 8 years from the date of final contact with the patient, or until the 25th birthday of a patient under the age of 18 at date of final contact. This is the recognised standard for the medical profession and is done purely for the purposes of the patient’s treatment and care.

The patient has a right to know the information Flawless Footcare retains on them. A request to access this information may be made free of charge in writing. A Request for Information template is available for the patient’s convenience upon request. Flawless Footcare will endeavour to provide the patient with this information within 5 working days of receipt of this written request.
